The Case Manager is an integral part of the clinical team here at Intellegis. The Case Manager works as the primary coordinator of the clinical efforts and as such schedules all medical appointments and referrals. The Case Manager is also the main point of contact for resident families and other stakeholders on the team. Every resident has a Care Plan developed by the Case Manager who is responsible for measuring outcomes and determining when new goals are appropriate. The Case Manager is the “hub” where all points meet in providing resident care.
